State mandated physical and dental exams are due to be completed and returned to the school nurse. If your child is in grades K/1, 6, or 11, and you have not returned a physical exam and updated immunizations, please have them completed and return no later than April 30. If your child is in grades K/1, 3, or 7 and you have not returned a dental exam, please have them completed and return no later than April 30. Forms are attached for your convenience.
