Responsibilities for District-Issued Device Use

  • PARENT/GUARDIAN and STUDENT agreement for the Greenwood School District to allow the STUDENT to use and possess the District Issued Chromebook, or any loaner or replacement device provided at the discretion of the District, for off district property hereby agree as follows:
     
    1. STUDENT and PARENT/GUARDIAN acknowledge receipt of Board Policy No. Policy 815: Acceptable Use of Technology Resources Student Use, Rights and Responsibilities and that they understand their applicability and agree to abide by the procedures, regulations and other rules set forth in them and in this Agreement. 
    2. STUDENT will not permit individuals, other than School District administrators, staff or teachers to access the Chromebook. 
    3. STUDENT and PARENT/GUARDIAN understand and agree that 
      1. the Chromebook is at all times the property of the District; and 
      2. STUDENT and PARENT/GUARDIAN has no right to disable or modify any hardware or software installed on the Chromebook. 
      3. Ensuring the device is fully charged at the start of the school day is the responsibility of the STUDENT. 
    4. STUDENT and PARENT/GUARDIAN understand and agree that the Chromebook is deemed to be in the custody of STUDENT from the time STUDENT receives the Chromebook until the time the Chromebook is returned to the designated School representative. 
    5. If the Chromebook and/or the charger or case are lost or stolen, PARENT/GUARDIAN and STUDENT will immediately advise Greenwood School District Administration in writing of the incident and all relevant information. 
    6. STUDENT and PARENT/GUARDIAN understand and agree that they are not to attempt any repairs/services on the Chromebook and that damaged Chromebooks must be returned to the Technology Department for repair/service. 
    7. STUDENT and PARENT/GUARDIAN understand and agree the right to use and possess the Chromebook terminates no later than the last day of classes during the school year unless terminated by the District or upon STUDENT withdrawal from the District. Failure to return the Chromebook and the continued use of it for non-academic purposes without District consent may be considered an unlawful appropriation of school property and authorities may be sent to your place of residence, or other location to take possession of the Chromebook and accessories. 
    8. If the Chromebook, charger, and/or case are lost, damaged or stolen, then STUDENT and PARENT/GUARDIAN agree that they are responsible to the School District for the cost to repair or replace the Chromebook and/or it's accessories at a cost not to exceed $300. 
    9. PARENT/GUARDIAN understand that the Parent/student has the option to participate in the district sponsored insurance plan to assist in the financial obligation of a damaged and/or lost or stolen Chromebook, charger, and/or case. 
    10. PARENT/GUARDIAN understand and agree that a refund of the insurance fee will not be issued for any reason, including but not limited to STUDENT withdrawing from the district, STUDENT privileges being suspended or terminated, and/or change of desire to participate in the insurance program. 
    11. PARENT/GUARDIAN understand and agree that an additional $35 charge may be issued for a returned check due to insufficient funds. 
    12. If the PARENT/GUARDIAN agree to participate in the Chromebook Insurance Program, then the following fee schedule will be applied. 
      1. Family Plan not eligible for free or reduced lunch status (3 or more children participating in 1:1 take home program) = $50
      2. Individual Plan Not Eligible for Free or Reduced Lunch Status = $20 
      3. Family Plan eligible for reduced lunch status (3 or more children participating in 1:1 take home program) =$25 
      4. Individual Plan Eligible for Reduced Lunch Status= $10 Eligible for Free Lunch Status = $0 
    13. PARENT/GUARDIAN understand and agree that requesting or providing insurance fee payment at the Free or Reduced Lunch Eligibility status provides the district with PARENT/GUARDIAN consent to access STUDENT's Lunch eligibility information. 
    14. If the PARENT/GUARDIAN participate in the Chromebook Insurance Plan, then PARENT/GUARDIAN agree to pay the following Insurance plan deductible schedule for the academic year regardless of Free/Reduced Lunch Eligibility Status:
      1. Accidental Breakage/Damage of Chromebook and/or accessories: 
        1. First Claim = $0 
        2. 2nd Claim = Half the cost of repair/replacement (not to exceed $150) and administrative consultation 
        3. Intentional misuse/damage of Chromebook and/or Accessories Full cost of repair/replacement (not to exceed $300) and discipline referral 
      2. Lost or Stolen Chromebook and/or accessories 
        1. First Claim = Half the cost of replacement (not to exceed $150) and administrative consultation 
        2. 2nd Claim=Full cost of the replacement (not to exceed $300) and administrative consultation 
        3. Damaged, Lost or Stolen Case First Claim = Half the cost of replacement (not to exceed $20) 
        4. 2nd Claim = Full cost of replacement (not to exceed $40) and administrative consultation
